GDPR – Bescherming van uw persoonsgegevens
Bij Mouvman hechten wij veel belang aan de bescherming van uw privacy en persoonsgegevens. Uw gegevens worden verwerkt in overeenstemming met de Algemene Verordening Gegevensbescherming (GDPR/AVG) en de toepasselijke Belgische wetgeving.

2. Welke gegevens verwerken wij?
In het kader van uw kinesitherapeutische behandeling kunnen wij onder meer de volgende gegevens verwerken:
• identificatie- en contactgegevens;
• gegevens die nodig zijn voor het beheer van uw afspraken;
• administratieve en facturatiegegevens;
• informatie verstrekt door uw arts of andere zorgverleners;
• gezondheidsgegevens die noodzakelijk zijn voor uw behandeling en de opvolging van uw patiëntendossier.
3. Waarom verwerken wij uw gegevens?
Uw gegevens worden uitsluitend verwerkt wanneer dit noodzakelijk is, onder meer om:
• uw afspraken te organiseren en beheren;
• uw behandeling en therapeutische opvolging te verzekeren;
• uw patiëntendossier bij te houden;
• met u of, indien nodig, met andere betrokken zorgverleners te communiceren;
• facturatie en administratieve verplichtingen uit te voeren;
• onze wettelijke en professionele verplichtingen na te leven.
Gezondheidsgegevens genieten een bijzondere bescherming en worden uitsluitend verwerkt binnen het kader dat door de toepasselijke wetgeving is toegestaan.
4. Met wie kunnen uw gegevens worden gedeeld?
Uw persoonsgegevens worden niet verkocht of voor commerciële doeleinden gebruikt.
Wanneer dit noodzakelijk is voor uw behandeling of wettelijk vereist is, kunnen bepaalde gegevens worden gedeeld met betrokken zorgverleners, ziekenfondsen, verzekeringsmaatschappijen of andere wettelijk bevoegde instanties.
5. Vertrouwelijkheid en beveiliging
Mouvman neemt passende maatregelen om uw persoonsgegevens te beschermen tegen ongeoorloofde toegang, verlies, misbruik of openbaarmaking.
Gezondheidsgegevens zijn uitsluitend toegankelijk voor personen die hiervoor in het kader van hun functie bevoegd zijn.
6. Bewaartermijn
Uw gegevens worden bewaard zolang dit noodzakelijk is voor uw behandeling en overeenkomstig de bewaartermijnen die voortvloeien uit de toepasselijke Belgische wetgeving inzake patiëntendossiers en administratieve verplichtingen.
7. Uw rechten
Overeenkomstig de GDPR beschikt u, onder de voorwaarden bepaald door de wetgeving, over het recht om uw persoonsgegevens in te zien en, waar van toepassing, deze te laten verbeteren, de verwerking ervan te beperken, bezwaar te maken tegen de verwerking of gegevens te laten wissen.

• tenir votre dossier patient ;

• assurer la facturation et les démarches administratives ;
• respecter nos obligations légales et professionnelles.
Les donnĂ©es relatives Ă votre santĂ© font l’objet d’une protection particulière et sont traitĂ©es uniquement dans le cadre autorisĂ© par la lĂ©gislation applicable.
4. Avec qui vos données peuvent-elles être partagées ?
Vos données ne sont pas vendues ni utilisées à des fins commerciales.
Lorsque cela est nĂ©cessaire Ă votre prise en charge ou imposĂ© par la loi, certaines informations peuvent ĂŞtre communiquĂ©es aux professionnels de santĂ© concernĂ©s, aux mutualitĂ©s, aux assurances ou Ă d’autres organismes lĂ©galement habilitĂ©s.
5. Confidentialité et sécurité
Mouvman prend les mesures appropriĂ©es pour protĂ©ger vos donnĂ©es contre l’accès non autorisĂ©, la perte, l’utilisation abusive ou la divulgation.
Les informations relatives à votre santé sont accessibles uniquement aux personnes autorisées dans le cadre de leur fonction.

Vos données sont conservées pendant la durée nécessaire à votre prise en charge et conformément aux délais de conservation prévus par la législation belge applicable aux dossiers de patients et aux obligations administratives.
